REIMBURSEMENT

• Chronic renal failure is reimbursed at 100% by the French Social Security system.
• Treatment costs are billed directly to your health insurance fund after prior agreement.
• In the event of hospitalisation, you may be asked to pay a fixed daily rate, depending on your social security cover.

DOCUMENTS TO BE PRESENTED

PATIENTS AFFILIATED TO THE GENERAL SOCIAL SECURITY SYSTEM IN FRANCE

• Identity document.
• Carte vitale.
• Mutual insurance card.
• Valid certificate.
• Certificate of CMU or AME cover.

PATIENTS FROM AN EU COUNTRY

• European health insurance certificate.

PATIENTS FROM OUTSIDE THE EEC OR NOT COVERED BY HEALTH INSURANCE

• Payment at the start of treatment.

COMPLAINTS AND CONSUMER MEDIATION

All complaints must be sent in writing to the Clinic's Quality Department: qualite.turin@almaviva-sante.com or by post: Clinique Turin - Service Qualité - 5 Rue de Turin - 75008 Paris. If the request for a complaint to the Quality Department fails within one month, the patient may submit the dispute with the Clinic to the mediator free of charge within a maximum period of one year from the date of the written complaint. The mediator will attempt, independently and impartially, to bring the parties together with a view to finding an amicable solution.

The mediator may be contacted by e-mail, online or by post. In addition to their full contact details (surname, first name, contacts) and the written complaint that they must have sent beforehand to the clinic's quality department in order to try to resolve their dispute directly, patients are encouraged to provide the following information to the mediator:

• The nature of the request
• A statement and description of the dispute
• All documents and factual evidence useful to the mediator's understanding and analysis of the case".
